If you have a patient who needs a fertility consultation, we do not require a referral form. In these cases, however, we do encourage physicians to contact any of our offices to talk directly to our reproductive endocrinologists to get a better understanding of their case.

When to Consider Referring Your Patients
After reviewing your patient’s case and conducting your own testing and evaluation, you might consider sending them to a fertility specialist in any of these situations:
- AMH (Anti-Mullerian Hormone) levels less than 1.0
- FSH (Follicle Stimulating Hormone) levels of 8.0 or above on day 3 of their cycle
- Patient age 35 or older who has been unable to conceive
- Patients who have had a tubal ligation
- Failure to ovulate after 3 or more medicated cycles
- Concerns with Male Factor Infertility
For new patients at Dallas-Fort Worth Fertility Associates, we review all medical records, evaluations, or treatments at their first visit. In order to expedite this process, please complete this form prior to sending the referral.

Fertility Testing for Females
In addition to routine ovarian reserve testing (FSH, estradiol, and AMH levels) and ultrasounds to observe follicles, we also provide diagnostic HSG (hysterosalpingogram) for analysis of the tubes after a tubal ligation, confirmation of tubal patency, and uterine shape in conjunction with an infertility diagnosis. After we receive your patient’s referral, we will contact your patient directly to schedule their appointment for testing. Results from all testing will be interpreted and sent to you upon completion.

Fertility Testing for Males
Male Factor Infertility accounts for 30-40% of all couples that have trouble conceiving. Because of this, we conduct testing on both the male and female patients that seek fertility treatment.
If there are concerns about male factor infertility with your patient, our labs can perform testing to determine count, motility, and morphology. This test is called a semen analysis. We perform semen analysis between the hours of 8:30am and 11:30am Monday through Saturday at our Dallas IVF lab. We will contact your patient directly for scheduling. Results of the analysis are available within 48 hours. These results will be sent to you upon completion.
